Step 1. Initial Sign up

Name
Legal Name (Last Name, First Name)
Phone
Email
Date of Birth
Username
Password (Must be at least 8 characters with 1 uppercase letter, 1 lowercase letter, 1 number, and 1 symbol)

 

 

Step 2. Personal Information

Profile Picture (Click on the Camera to Add)
Address (Please include at least one physical address as your default address)
Emergency Contact
Optional Fields:
Symbility ID
Xactnet Address

 

 

Step 3. Work Information

Total Work History (Click on the pencil at the top right)
Resume
Claims Experience

Optional Fields:
Title
Type of Work Interested In
Software Experience (Recommended)
